This practice uses alternative medicines including but not limited to: acupuncture, homeopathy, Chinese herbal medicine, Western herbal medicine, ozone therapy, ultraviolet light therapy, laser therapy, and red-light therapy in addition to traditional western medication. I understand that the above alternative medications may be used in lieu of traditional western medications; such as steroids, antibiotics, antifungals, etc. Bloodwork, vaccines, vaccine titers, and exam data may be used for research/scientific studies/publications.

In accordance with Virginia Law § 54.1-3411.1. No prescription medications can be returned to clinic after purchase

Financial Responsibilities

-A $25.00 same day fee will be applied to any same day appointments. - A $100 emergency fee will be added for same day emergency cases. - A $25 cancellation fee will be applied to any appointments cancelled within 24 hours of the appointment -A $85 no show fee will be applied if you do not show to your appointment -A $25.00 late fee will be applied if you are late to your appointment -A $50 late fee will be applied if you are more than 30 minutes late to your appointment, and you will be rescheduled if we are not able to work you into the schedule. • ALL FEES ARE DUE UPON COMPLETETION OF VETERINARY SERVICES Animals First Veterinary Service will not be accepting checks. I understand that an interest rate of 1.5% per month will be charged on any past due amounts. If collection proceedings are initiated for any past due amounts, the client agrees to pay all said attorney fees. If it is necessary for us to engage the services of an attorney in regard to any violations of disputes regarding any other terms of this agreement, the client will be responsible for all of said attorney fees/costs. Any dispute as to the nature of this contract to the property venue shall be in Culpeper County, VA. The undersigned hereby waives the benefit of the homestead exemption with the regard to this transaction. The company is hereby given, and hereby retains, a lien on the clients’ files and documents, to the extent of any unpaid balances owed to a Company by Client.
